Where an incident has occurred, any organisation should strive to review the incident, learn lessons and implement any necessary changes to increase their level of resilience and to improve their response and recovery. There are occasions where an independent investigator is best placed to dispassionately review the circumstances of the incident and identify where enhancements to processes and systems should be implemented.
